Job Summary:
Legacy Healthcare LLC is seeking a compassionate and skilled Registered Nurses, Licensed Vocational Nurses, and Home Health Aides to join our Hospice team in Manteca and Modesto. As an RN, LVN, or HHA, you will provide high-quality care and support to patients with life-limiting illnesses in their homes, assisting them to maintain their comfort, dignity, and quality of life. Our team is dedicated to providing exceptional care and support to our patients and their families, and we are looking for a dedicated and compassionate individual to join our team.

Responsibilities (RN & LVN):
- Provide skilled professional nursing care to hospice patients
- Oversee and provide primary care, including all clinical care, palliative care, and symptom control as well as identify physical, psychological, social and spiritual needs for assigned patients.
- Communicate with other members of the interdisciplinary team to ensure all patient and family care needs are met.
- Work in collaboration with the Interdisciplinary Team in the planning, implementation, and evaluation of medical and nursing care.
- RNs establish a Care Plan to meet their needs, implement interventions and evaluate the patient and family's response to the Care Plan to provide appropriate comprehensive and responsive care plans.
Requirements (RN & LVN):
- Graduate of an accredited nursing program
- Current California Registered Nurse/Licensed Vocational Nurse license
- Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation (mileage reimbursement provided)
- Previous experience in hospice preferred
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Ability to lift up to 25 pounds and assist with transfers and ambulation as needed
- Reliable mobile phone with a plan that provides good local coverage.
- Basic computer skills
Responsibilities (HHA):
- Provide personal care and assistance to patients with daily living activities, such as bathing, dressing, grooming, and toileting
- Assist with meal preparation and feeding as needed
- Provide emotional support and companionship to patients and their families
- Assist with light housekeeping and laundry tasks as needed
- Monitor patients' conditions and report any changes or concerns to the hospice team
- Maintain accurate and timely records of patient care and services provided
- Collaborate with the hospice team to develop individualized care plans for patients
- Provide education and support to patients and their families on disease management, symptom management, and end-of-life care
Requirements (HHA):
Current certification as a Home Health Aide (HHA) or ability to obtain certification within 30 days of hire
High school diploma or equivalent required
Previous experience in hospice or home health care preferred
Ability to work independently and as part of a team
Excellent communication and interpersonal skills
Ability to lift up to 25 pounds and assist with transfers and ambulation as needed
Valid driver's license and reliable transportation required (mileage reimbursement provided)
